As soon as your death file has been compiled, give a copy to your Primary Care Physician, tell your Medical Decision Maker(s) and 2 other close family/friends where to find it. You can also upload to mydirective.com. If you change its location or any of the directives in the form, be sure to update everyone as is appropriate.

There are many state-specific forms available to download for free. A good place to start is to ask your doctor for the version they recommend to patients.
I personally recommend the Five Wishes form. This form is available in 32 different languages & meets the legal requirements for advance directives in 42 states + D.C. You can preview a protected PDF copy here (not printable). Both print and digital copies are available for purchase at the Five Wishes organization; a printed copy of the form costs $5 and is mailed to you. You can purchase a copy here.
